Passed away peacefully on Monday September 17, 2018 in Toronto, Ontario. Beloved wife of Kok Ming. Loving mother of Leo, Frank and Sarah (Sing Wah). Caring grandmother of Louis, Henry Joakim, Holly, Lilly, Yat Lai and Yat Mei. Cherished great grandmother of Victor August and Theodore Alexander. Polly will be sadly missed and fondly remembered by all her relatives and friends.
Friends may pay final respect to Polly on Tuesday September 25, 2018 at Chapel Ridge Funeral Home, 8911 Woodbine Avenue in Markham (4 traffic lights north of Highway 7, on the east side of Woodbine Avenue). Visitation will be from 8:30 am - 09:45 am. A Buddhist Service will commence at 09:45 am to be followed by cremation at Highland Hills Crematorium. (12492 Woodbine Avenue, Gormley, Ontario). A Reception will be held at Dragon Legend Buffet (25 Lanark Road, Markham, Ontario) right after cremation.
